Care Coordinator
POSITION TITLE: Care Coordinator
PROGRAM: South County Academy
LOCATION: Exeter, RI FULL TIME
Summary: Responsible for the health care coordination of every student at SCA. The central function of this position is appointments. The Care Coordinator schedules, prepares for, staffs, tracks, and documents all medical, dental, vision, psychiatric, and specialty appointments for every student from admission through discharge, and ensures that nothing that is ordered, recommended, or due is missed. Carries the program’s Building Bridges Initiative (BBI) contact responsibilities, maintaining active communication with families, guardians, DCYF, and identified permanency connections so that the people who will be in a student’s life after discharge are involved in the students’ care while here. Advocates for students and families in navigating human service departments, agencies, and services, and collaborates with DCYF, schools, medical providers, and other service providers to ensure appropriate delivery of services. Supports the maintenance of Trauma Systems Therapy in the milieu setting.
Qualifications:
· Bachelor’s degree in social work or a related human services field, or significant residential or childcare experience, required.
· Experience coordinating medical care, scheduling, or case management strongly preferred. Experience in a licensed residential or health care setting preferred.
· Demonstrated organizational skill and reliability with tracking systems, calendars, and deadlines. This position fails if appointments are missed.
· Knowledge of Rhode Island Medicaid, EPSDT requirements, and DCYF health care processes preferred.
· Willingness and capability to work flexible hours including evenings and weekends as scheduled.
· Knowledge of and sensitivity to cultural, ethnic, racial, and socioeconomic issues.
· Excellent written and verbal communication skills.
· Ability to maintain confidentiality and to comply with HIPAA where applicable.
· Bilingual in English and Spanish preferred.
· Must pass all background checks required for employment in a licensed residential program serving youth, including BCI and DCYF clearances.
· Bilingual/ASL skills are compensated by an additional 6%, above base pay.
· Multilingual skills are compensated by an additional 8%, above base pay.
Physical Requirements: This position requires residential and community visits, employees in this position must have the ability to:
· Travel to and from clients’ residence, community locations and office site, which could include using walkways, stairs and/or elevators.
· Ability to lift up to 20lbs.
· Ability to communicate by voice, use sight and hearing.
· Ability to push or pull 10 lbs or more
· Ability to pick up 10 lbs or more
