POSITION TITLE: Data Coordinator

PROGRAM: CCBHC IA

LOCATION: Hope St 

FULL TIME: Full Time 

SALARY RANGE: $24.14 - $30.02 Hourly

Summary: Responsible for the oversight of project data collection, entry, and management to support client care. Collaborate with Directors, Supervisors, and team members on data requirements and collection for federal and state grant reporting. Coordinate with Billing Department and CCBHC team members to ensure data integrity. Support on-going fidelity of the CCBHC framework through monitoring of metrics and benchmarks.

SPECIFIC DUTIES/RESPONSIBILITIES:

· Assists with the development and implementation of the NOMs workflow to ensure all baseline, reassessment, and discharge NOMs are collected in a timely manner per federal requirements.

· Manually enter all NOMs data into SPARS according to the required timelines.

· Provides regular monitoring of NOMs data timelines for baseline and follow-up assessments

· Conducts direct interviews with clients for GPRA data collection and communicates with staff around timing and compliance of GPRA data completion.

· Ensure that Attribution Portal remains up to date by tracking eligibility and attribution of all CCBHC clients from initial evaluation, across services/levels of care, to discharge and entering all relevant data into the State Medicaid Portal.

· Share information from attribution data review with program staff, Quality Management, & Billing teams to maximize efficiency & revenue.

· Ensures BHOLD data remains up to date by developing training materials, training new staff, checking that required services are entered, supporting supervisors in quality assurance, and manually entering all data into BHOLD database.

· Monitor compliance of CCBHC data across grant, state and federal requirements

· Proficient in using an electronic health record as well as project-specific software.

· Complies with FSRI policies, including those relating to HIPAA and CCBHC

· Complete all required documentation within required timeframes.

· Collaborate with CCBHC team members.

· Participates in all meetings as required by the CCBHC team, supervisors and necessary for the role both internally and externally.

· Maintain confidentiality of clients at all times.

· Attend and participate in all required program and agency trainings.

· Approaches all work through a trauma and diversity and inclusion lens.

· Performs additional responsibilities consistent with Agency needs, defined in consultation with supervisor(s)

Qualifications:

· Bachelor’s Degree required.

· Experience in human services preferred.

· Skilled in operating various medical record software and hardware, word-processing, and database software programs

· Spanish Speaking preferred and is compensated

· Flexible schedule with some evenings.

· Excellent multitasking and communication skills a must

· Ability to work independently and part of a team.

· Bilingual/ASL skills are compensated by an additional 6%, above base pay.

· Multilingual skills are compensated by an additional 8%, above base pay.

PHYSICAL REQUIREMENTS: This position requires community visits, employees in this position must have the ability to:

· Current driver’s license, reliable transportation, registration and auto insurance.

· Ability to communicate effectively.

· Travel to and from clients’ residence, community locations and office site, which could include using walkways, stairs and/or elevators.

· Ability to lift up to 20lbs.

· Must be able to work remotely and in person adhering to PHI requirements.
