Position Summary: The CHW coordinates families connecting to services and assists families in ensuring basic needs are met, helping to fill out benefit forms, connect to legal services, traverse a complicated immigration process, access food banks, ensure each member of the family establishes a medical home, and with navigating the educational system— helping children entering public school systems fully vaccinated, as well as facilitating referrals to supports for special education services. During visits, the CHW works with the family to help them build skills such as: addressing safety concerns, accessing routine and preventive medical care, caring for infants and children in early childhood, identifying and meeting their social-emotional needs, recognizing and encouraging developmental and educational milestones of the child(ren), identifying gaps in care and ameliorating those gaps, supporting caregivers in worker and/or service transitions, and advocating to ensure that care is adhered to for the child.

 

Qualifications:

  • Associate or Bachelor’s degree preferred, High School Diploma or Equivalent required.

  • Certified Community Health Worker, or ability to be certified within first 6 months of employment.

  • Experience working with the childhood and adolescent population.

  • Bilingual in Spanish preferred

  • Valid driver’s license and reliable transportation.

  • Ability to document in and navigate an Electronic Medical Record (EMR).

  • Strong interpersonal and social skills with an ability to collaborate with a variety of individuals.

  • Ability to provide services virtually, in resident’s homes, and community locations.

  • Ability to have a flexible schedule to include evening shifts and occasional weekends.

  • Prior experience in behavioral health and/or case management preferred.

  • Bilingual skills are compensated by an additional 6%, above base pay.

 

Physical Requirements: This position requires agency and community visits, employees in this position must have the ability to:

  • Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.

  • Ability and willingness to utilize required Personal Protective Equipment as required.

  • Follow FSRI professional dress code.

  • Ability to lift up to 20lbs.

  • Ability to communicate effectively.
