POSITION TITLE: Coordinator, Cedar 

PROGRAM: CEDAR 

PART TIME 

SALARY RANGE: $19.00 - $23.62 Hourly

Summary: Serves as the initial point of contact for clients and families seeking assistance through Family Service of Rhode Island’s Cedar Program. This role is responsible for conducting intake assessments, completing required screenings, obtaining necessary documentation from medical providers, and making appropriate referrals to internal or external resources. Ensures timely, and accurate support to clients by gathering essential information, maintaining detailed records, and working collaboratively with other service providers. Documentation and training as required by the certification and organizational standards.

Qualifications:

Experience working children and families in the community from diverse population.

Experience working with children and families with special health care needs preferred

Rhode Island Community Health Worker Certification preferred; certification required within the first 12 months of employment

Excellent communication skills

Ability to utilize an Electronic Medical Record and familiarity with Outlook, Word, Excel

Ability to work in a hybrid environment while maintaining organizational PHI standards and confidentiality

Reliable transportation, current driver’s license, proof of automobile insurance and automobile registration

Physical Requirements:

Travel to and from community and agency site locations, which could include using walkways, stairs, and /or elevators

Ability to lift 20 pounds

Ability to communicate effectively
