We provide services statewide, and currently operate in three locations in Providence; and four locations in East Providence, Smithfield and North Smithfield.
Summary: CHW will support SPHEZ project initiatives, including community engagement and bringing resident voice to Steering and Subcommittee meetings and activities, community event organization, youth engagement, resident focused data collection on needs, and other place-based project activities. The CHW will focus on interventions to improve individual and community wellness based on ongoing feedback. CHW is responsible for engaging residents and community stakeholders; implementing surveys and other screening tools; identifying areas of need; ensuring referrals are made to appropriately matched services; and supporting individuals with accurate, up to date health in prevention resources. CHW will work primarily in the Providence 02905 zip code through a variety of settings, including virtually, on-site in offices, in resident homes, and in other community venues. The CHW will work collaboratively across FSRI and external community-based programs and initiatives as needed.
Qualifications:
Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required.
Certified Community Health Worker, or ability to be certified within first 6-12 months of employment.
Bilingual in Spanish required.
Valid driver’s license and reliable transportation.
Candidates residing in South Providence are strongly encouraged to apply.
Ability to document in and navigate an Electronic Medical Record (EMR).
Strong interpersonal and social skills with an ability to collaborate with a variety of individuals.
Ability to provide services virtually, in resident’s homes, and community locations.
Ability to have a flexible schedule to include evening shifts and occasional weekends.
Prior experience in behavioral health and/or community health preferred.
Ability to work virtually and use telehealth.
Bilingual/ASL skills are compensated by an additional 6%, above base pay.
Multilingual skills are compensated by an additional 8%, above base pay.
Physical Requirements: This position requires agency and community visits, Employees in this position must have the ability to:
Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.
Ability and willingness to utilize required Personal Protective Equipment as required.
Follow FSRI professional dress code.
Ability to lift up to 20lbs.
Ability to communicate effectively.
