POSITION TITLE: Community Services Program Coordinator
PROGRAM: TST
FULL TIME: 40 hours per week
SALARY RANGE: $19.75 - $24.59 Hourly
Summary: Works as an essential part of a dynamic and creative team to ensure quality service implementation and support the treatment goals for youth (ages 4 to 19) and their families who are engaged in intensive, trauma-informed home/community-based services. The Community Services Program Coordinator assists the Department Director and team to meet required goals, objectives, deliverables, and financial targets of new and ongoing initiatives. Participates in internal and external meetings focused on service delivery relevant to youth referred to the services by DCYF, FCCP, CCBHC or the court system. Ensures compliance with quality standards of all federal, state, and administrative body laws and regulations including DCYF. Advocates for the youth and families to access appropriate and necessary resources to improve the quality of their lives. Manages the team’s schedules and collaborates with referral sources and other providers (schools, psychiatric providers, and other agencies) involved in the care of the youth to ensure treatment meetings are established and consistent.
Qualifications:
- Bachelor’s degree in a human services field required
- Experience in project development, management, data and evaluation
- Highly advanced critical thinking skills, creativity, flexibility in meeting changing priorities and demands; ability to think in broad and detailed terms; keen attention to detail; comfortable with identifying and resolving obstacles to improvement/goals
- Skilled in operating various medical record software, word-processing and excel
- Spanish Speaking preferred and is compensated
- Ability to work independently and part of a team
- Bilingual skills are compensated by an additional 6%, above base pay.
- Multilingual skills are compensated by an additional 8%, above base pay.
Physical Requirements: This position requires residential and community visits, employees in this position must have the ability to:
- Current driver’s license, reliable transportation, registration and auto insurance
- Ability to communicate effectively
- Travel to and from clients’ residence, community locations and office site, which could include using walkways, stairs and/or elevators
