Reports To: Director of Behavior Health & Social Services Department: Social and Behavioral Health Services Job Grade: Employment Status: Exempt

General Responsibilities: To apply knowledge of methodologies of psychotherapy in order to provide individual, group, and family behavioral health counseling and assessment services to assigned patients. Required Qualifications: 1. Master’s degree and appropriate license/certification in social work and two years clinical experience. 2. Experience with diverse populations and bi-lingual/bi-cultural candidates (Spanish/English) preferred.

Physical Effort/Environment: Moderate physical activity is required: sitting, standing, walking, and lifting. Travel to off-site meetings and other program sites as needed is required. Work Schedule Demands: The work schedule is 40 hours per week, mornings, afternoons, and evenings as required.

Communication Skills: 1. Strong communication skills, both oral and written, required. 2. The Ability to effectively work and communicate with a multi-disciplinary team, other services providers, and patients is critical to the success of this position. 3. The Development and submission of written reports accurately detailing service provision is required.

Confidentiality of Information: The incumbent has access to client’s medical records, financial information, and documentation regarding prior services from outside agencies. The maintenance of client information confidentially is required.

Significant Job Functions: Clinical & Direct Service Responsibilities 1. Apply knowledge of Psychiatric Diagnosis (DSM-IV-TR & ICD-9) in order to perform and record mental status examination findings and multi-axial diagnostic formulation with assigned patient caseload. 2. Assess clinical emergencies, including risk of suicide and danger to others. 3. Apply knowledge of methodologies of psychotherapy in order to provide individual, group, and family behavioral health counseling. 4. Collaborate with other providers, agencies, and individuals in the patients’ network of care. 5. Communicate with various local agencies and individuals, as well as with Center staff. 6. Provide patients with self-management skills and educational information needed so they can be full participants in their own treatment and recovery. Documentation & General Administrative Duties 1. Perform timely and accurate record keeping in accordance with all Health Center requirements. 2. Complete daily activity logs, behavioral health screening & progress notes, encounter forms, reports, evaluations, and insurance documentation as required by pay sources. 3. Complete clear, specific, and behaviorally measurable treatment plans with assigned patients. 4. Maintain cooperative relationships with Health Center employees, patients, community agencies and the public. 5. Work in cooperation with co-workers and supervisory staff. 6. Deal with team members regarding problems in a constructive manner. 7. Maintain a willingness to listen to and respond to constructive feedback from supervisory personnel. 8. Serve on Health Center committees and workgroups as requested. 9. Perform other duties as assigned for which you are qualified.
