Full job description

Transitional Housing Program Coordinator and Case Manager is the primary Case Manager for all residents of Amos House Men's Transitional 90 Day Shelter. The coordinator is responsible for intakes, assessments and all case plans for all residents in the program. Coordinator will also coordinate with other agencies and organizations such as the ACI, Providence Center, detox facilities and hospitals for referrals to Amos House. The coordinator will work with probation and parole officers as needed for any individual who comes to the Amos House program as a condition of probation or parole.

Coordinator also supervises the House Managers, including hiring and terminations, managing scheduling, days off, sick time, holidays, supervising their day-to-day interactions with the guests, and handling crisis as they arise.

Coordinator holds a weekly house meeting, attended by staff and guests to address community living issues, including conflicts among men, chores, meals, etc.

Coordinator will be responsible for all tracking and reporting of initial interviews for guests, case plans, Clarity and AWARDS data entry, income tracking and updates, and will also be "On Call" for the Men's Program after regular business hours, weekends and holidays.

The coordinator will bill Medicaid for eligible guests for CPRS/CHW or Home Tenancy services.

As guests complete the 90 Day Transitional Shelter Program and move into Amos House housing, they will transfer from the Transitional Housing coordinator to the case load of the Phase II coordinator.

The Program Coordinator/ Case Manager will meet with each guest on campus at least one time per week.

Duties and Responsibilities:

  • Crisis Intervention, as needed, including referral to detox in the event of re-occurrence.
  • Ongoing orientation and education regarding the rules and expectations of the 90 Day Program
  • Supervision of attendance at AA/NA meetings
  • Case Plans, Progress Notes, Releases of Information and records of employment and medical/mental health information. Program coordinator will report on progress of each guest during weekly supervision meeting with the clinical director. Coordinator will document progress in Clarity as well as AWARDS and will bill for services in AWARDS with back-up case notes that match treatment/case plans.
  • Coordinator will facilitate at least 3 groups per week in the Day Program; including Uplift and other topic discussion meetings.
  • Coordinator will meet 1-1 with each House Manager every week to review their performance in role of House Manager and work on any corrective actions needed in order to ensure the best treatment possible for our guests.
  • Coordinator will meet with the clinical director at least one time per week for 1-1 supervision.

Qualifications:

  • The Coordinator of the Men’s Program must have a license in Substance Abuse Disorders (LCDP) or a BSW/MSW with extensive experience with individuals with Co-Occurring Diagnosis.
  • Candidate will have worked with unhoused, formerly incarcerated or other impacted group of similar nature.
  • Persons with lived experience with at least 12 consecutive months of sobriety will be considered with a certification in Peer Recovery Services (CPRS) and related degree.
  • Candidate must have computer literacy skills to enter data in software systems.
  • A valid driver’s license is required.
  • MSW/LCSW or LMHC preferred.
  • Bi-lingual candidates are encouraged to apply.
