Currently seeking an energetic Residential Case Manager to assist in the delivery of psychiatric, rehabilitation, and supportive services to adults living in our supervised apartments in Woonsocket.

The Residential Case Manager will provide a safe, stable space for those clients with mental health challenges to help them achieve personal goals and improve quality of life. An ideal candidate will work with clients to help them develop social and daily living skills and work to address their needs.
Duties and Responsibilities:

  • Provides case management services to residents in accordance with the client's individual treatment plan, goals, philosophy, and standards defined by the agency.

  • Continually assesses and evaluates clients, providing support services, crisis intervention, medication monitoring and education on as-needed basis.

  • As a Case Manager you will educate clients, their families and significant others regarding mental health issues, substance abuse and the impact on their lives.

  • Facilitates referrals to primary health care providers and specialty care providers (health and behavioral health) in the community as well as within the agency and provide support as necessary.

Education/ License Requirements:

  • Associate's Degree in Human Services with some experience is a requirement; BA/BS is preferred.
  • Valid drivers license, auto insurance and reliable vehicle.
  • CPR/AED & First Aid required. If you do not have this certification, you can attend one of the classes that isprovided free of charge by CCA.
