POSITION SUMMARY:

This position will provide case management and direct support to individuals residing at the Smithfield Shelter. The case manager will provide services to assist residents in stabilizing their situation and exercising a successful transition to independent and self-sufficient living. This individual will provide resources and advocacy to individuals while working on a housing plan. The Shelter Case Manager will provide shelter-based services establishing and maintaining a service plan that guides their involvement in the program. The case manager will be expected to connect the family to needed services and coordinate those services and referrals guided by the case management plan.

EDUCATION AND TRAINING REQUIRED:

  • Associates degree required in Human services or related field.
  • Some relevant experience preferred.
  • Ability to work independently and within a team. Interest in ongoing training, supervision, and professional development.
  • Bi-lingual skills are welcome.

WORKING CONDITIONS:

  • Provide services in shelters, in the community, and within the agency.
  • Maintain a valid driver's license and own reliable transportation.
  • Work in a team environment.

SPECIFIC FUNCTIONS:

  • Deliver case management services to assigned clients within the shelter and any related programs as assigned by supervisory staff.
  • Complete all required documentation promptly.
  • Carry a caseload of 12-15 individuals as needed.
  • Provide referrals/ care coordination.
  • Assist in room inspections, floor meetings, and conflict resolution.
  • Keep appointments with clients in the office.
  • Attend team meetings, agency-wide meetings, and community meetings as assigned.
  • Conduct case conferences related to client progress.
  • Assist in all Performance Quality Improvement efforts related to shelter programs.
  • Other duties as assigned by supervisors.
