East Bay Community Action Program (EBCAP) has a part time opening (10 hours per week including one evening) for an Independent Licensed Clinician to provide a wide range of clinical services and support EBCAP's Transgender Whole Health Care program based in our Newport location.

The Clinician will facilitate the following:

Patient engagement and outreach via phone, and letter.
Conduct brief treatment (Motivational Interviewing, Behavioral Activation, Coaching, Solution focused treatment, psychoeducation, relapse prevention, crisis stabilization, etc.).
Complete ongoing assessments utilizing standardized instruments such as PHQ-9, AUDIT, CAGE, GPRA, PSC-17 etc.
Track and enter treatment response into electronic medical record.
Meet with consulting psychiatrist, integrated healthcare team, Same Day Access team, Transgender Whole Healthcare team, and other relevant care teams for patient/record review.
Complete mental health support process for patients seeking medical gender affirmation, including detailed social and gender history, gender affirming mental health interventions, and referral letter writing.
Complete referrals to traditional internal and external Behavioral Health programs when applicable.
Assist Emergency Services team with crisis stabilization, opiate triage, coordination of MAT inductions, harm reduction education and materials.
Coordinate with other TWHP team members around SDOH, personal, and medical needs of all assigned TWHP clients to ensure well-rounded, person-centered care and solutions.

Masters’ Degree in Human Services field; independently licensed (LICSW, LMHC, LCDP) required.
