East Bay Community Action Program (EBCAP) is seeking a full time Case Manager for our Education and Career Pathways department at our Grove Avenue location in East Providence, RI.

How you will make an impact!

  • Complete assessments, including but limited to: Family Development, academic (using CASAS), and vocational/employment factors with assigned clients.
  • Develop a service plan and a set of client goals with each client in accordance with provisions dictated by Workforce Development team leadership, program design and/or funder obligations.
  • Process appropriate releases of information to obtain education, vocational, medical/behavioral health records, etc., that may relate to, and/or impact an individual’s ability to succeed in the program.
  • Provide coaching as needed and as related to barriers for academic/vocational/employment goals.
  • Assist with the completion of applications for internal and external community resources for clients as needed.
  • Maintain regular client contact as well as timely and accurate recordkeeping for client files in accordance with program, including but not limited to: documenting in LACES (Literacy, Adult and Community Education System), and CODECT as deemed appropriate by supervisor and program/funders (e.g. RIDE, DHS or DLT).

What you will bring:

  • Bachelor's Degree in a Human Services field or related field,
  • Minimum of 2 years related experience required.
  • Bi-lingual in Spanish a plus.
  • Must have demonstrated case management skills and experience; emergency intervention methods, family dynamics, health, mental health and substance abuse systems; and knowledge of community resources.
