General Summary: Coordinate day-to-day functions of Access Services. Directly Oversee the Access Coordinator(s) and Care Coordinator. Provide support and direction to the Case Manager, Peer Support Specialist and Benefit Specialist as needed. Coordinate the scheduling of all Assessment appointments including Open Access. Assist the Access Program Manager as needed. Responsible to coordinate all DCO referrals and discharges.

Essential Responsibilities:

  • Ability to cover duties of Access Coordinators, Care Manager, Patient Registration and Benefit Specialists as needed.

  • Manage the CCBHC Client Portal for Access

  • Facilitate all Referrals from CCBHC DCOs, as needed.

  • Troubleshoot problems regarding Thrive referrals and related system issues.

  • Interface regularly with Thrive programs and support staff regarding clients/referrals related to ACCESS.

  • Maintain current knowledge of all Thrive CCBHC services, including DCOs.

  • Create and maintain team with positive role modeling.

  • Carry and enliven a positive Thrive image consistent of Thrive principles.

  • Maintains professional relationship with representatives in Thrive, public officials, community leaders and the general public.
  • Maintain and enhance the Thrive's culture of learning and growing by serving as a role model, mentor, facilitator, educator, and consultant to Thrive staff on program and service issues.

  • Maintain overall information and data about functioning of Access Services.

Additional Responsibilities:

  • Attend all agency/department required training and meetings within the prescribed period.
  • Attend all Safety Training programs and be able to describe responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.

Qualifications

Education, Experience and Competencies:

  • Associate’s Degree in Human Service or related field, or equivalent experience required.

  • At least two (2) years experience in the Human Service field required.

Certifications, Licenses Requirements:

  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.
